For new patients, you should plan to arrive at least 15 minutes before your appointment. Please remember to bring:
- Insurance card(s); if you have a second insurance please bring both cards.
- Current medications and dosages.
- Medical records, if you have them (especially immunization records).
- Referral from your physician (if required).
- Any copay required by your insurance.

You should do your best to keep your appointment. If a change is necessary, please call your physician's office 24 hours before the time of your appointment whenever possible. Another patient may need your appointment.
Registration
When you arrive, you will speak to a Patient Services Representative. He or she will ask for your name, address, telephone number, social security number, place of business, emergency contact and insurance information. To speed up the process for new patients, fill out our Patient Information Form and have it with you at your appointment.
- Patients who have job related injuries and have filed a Workers' Compensation claim must bring a letter from their employer at the time of their visit, along with the name, address, telephone number of their Workers' Compensation insurance company and claim number, if available.
- If your medical problems are from a motor vehicle accident, you will be expected to pay for your visit in full at the time of service. You can be reimbursed for these costs from the responsible auto insurance carrier directly.
- If you have retained an attorney for your medical problems, you need to provide the name, address, and telephone number of the attorney at the time of your visit.
Payment
We request payment at the time of your appointment for services which are not covered by insurance. Your insurance company may also require you to pay a co-payment at the time of your appointment. For your convenience, Mastercard, Visa, personal checks, money orders and cash are accepted.
Obstetrical fees not covered by insurance are to be paid in full by the 8th month of pregnancy. Surgical fees will be discussed with you.
Patient Billing
You will be sent a single monthly statement for all services provided by one of our medical professionals. All balances are due in full upon receipt. If you have any payment concerns or questions regarding your account, please contact our billing office at (860) 258-3480 Monday through Friday from 8:30a.m. to 5:00p.m.
Insurance Billing
Connecticut Multispecialty Group physicians accept Medicare, Blue Cross Blue Shield, and most major insurance plans. You are responsible for all balances due after your insurance company pays its portion. You should check with your insurance plan to see if the doctor you wish to see is a participating provider with that plan. If you wish to change your Primary Care Physician (PCP) to one of our Physicians, please call the office directly to set up an introductory visit.
